Amy is licensed as a Licensed Clinical Social Worker (LCSW) in the state of Illinois and can treat patients that currently are residents of the state of Illinois. Additionally, Amy holds a CADC (Certified Alcohol and Other Drug Counselor) certificate in the state of IL.

Amy is in-network with BCBS-IL PPO and PPO Blue Choice policies. Please refer to your medical insurance for costs per session as this differs from policy to policy. If you are out of network with BCBS-IL, self-pay rates apply.

At this time, Amy is not in-network with BCBS HMO & Community plans.
